Family Referrals to Hospice
Anyone can make a referral to Bella Care Hospice….a family member, friend, neighbor, pastor or even the patient themselves. A doctor’s order is not needed to make a referral for hospice care, so call us. When you call our Referral Coordinator, they will assign a registered nurse (RN) from the admissions team to come out and visit the patient.
During the first visit, the RN will:
- Explain the full range of programs and services
- Explore the patient’s situation and needs
- Determine the services which best fit the patient’s needs
- Talk with the doctor about your care needs
- Have the patient sign a consent form
- Assign the patient to a care team
- Determine what medications, supplies and equipment will be provided by Bella Care Hospice
It is really that easy.
